Ambulance Call Reports
Reports
The Paramedic Service keeps records (called Ambulance Call Reports) of interactions they have with their patients. Patients and their lawyers or insurance companies can request these records. Reports cannot be released without the signed consent of the patient.
These reports are kept for a period of five years. A report will include notes from the time that the ambulance arrives until the time that the patient leaves paramedic care.
How to request a personal report
Call the Paramedic Administration office at 519.44BRANT(519.442.7268), 1.855.44BRANT to make your request. For pick up, please attend the office at 303 Henry Street, Brantford with photo identification.
This service is free of charge for patients requesting their own personal report.
Law firms and insurance companies
Send us a letter on company letterhead with the following information:
- Patient's name
- Patient's date of birth
- Date, time and location of the incident
- Law firm file number
- Any other details that may be relevant
- The signed authorization form from your client
- Payment
Law Firms and Insurance companies are required to pay a fee when requesting an Ambulance Call Report. A cheque must be made out to the County of Brant Ambulance Service for each report required. Visit our Fees and Charges webpage for payment information.
