The Brant-Brantford Paramedic Service is one of five services in Ontario
to pilot a new program for Long Term Care.
The Community Paramedics Long Term Care program runs 24/7.
This program focuses on providing care to residents who are:
- On a LTC waitlist
- Soon to be on the LTC waitlist and/or
- Opted to remain in their own homes
The program goals include:
- Provide supplemental supports to provide safe and prompt care for matters not requiring treatment at a hospital
- To ease pressure on LTC waitlists while allowing people to remain in their own homes
- To Assist with caregiver burnout
The program target population:
- Patients requiring supplemental Community Paramedicine services, beyond home and community care
- ED Admission Avoidance for LTC bound patients
- Hospital Discharge (e.g. ALC to Home Care) for LTC bound patients
- Patients wait-listed for long-term care residing in the community (or soon to be active with LTC); may be at risk of requiring hospitalization and becoming ALC designated
- Citizens who may otherwise benefit from community paramedic involvement
