Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care.

Cancer Screening Mobile Coach

For anyone unable to access screening elsewhere, come to the Mobile Coach, all that is needed is a health card or Interim Federal Health Benefit.

Offer social, recreation and therapeutic activities for individuals with disabilities

Dedicated to providing essential programs and services designed to optimae the lives of individuals living with dementias and their care partners 

Complex Care-Brant Community Healthcare System 

People with complex care needs are admitted to the program when their care needs should be addressed in a multidisciplinary hospital setting. Complex care patients often need highly-skilled, technology-based care.

Stedman Community Hospice

We know that it is often a very difficult and emotional decision for a patient or family member to know when to contact the Hospice for care and support.  We want everyone to know that we are here for them, regardless of age, illness, race or economic status.  In fact, it is our hope that people contact the Hospice from the very moment of diagnosis of a life-limiting illness, even if they may have many years to live.  

VON Safety at home Assessment:

Provides free home assessments and recommendations to help older adults stay safe in their homes.
