The Paramedic Service keeps records (called Ambulance Call Reports) of interactions they have with their patients. Patients and their lawyers or insurance companies can request these records. Reports cannot be released without the signed consent of the patient.

These reports are kept for a period of five years. A report will include notes from the time that the ambulance arrives until the time that the patient leaves paramedic care.

How to request a personal report

A request form application is available, online. Please fill out the request form below:

This service is free of charge for patients requesting their own personal report.

Law Firms and Insurance companies are required to pay a fee when requesting an Ambulance Call Report. Payment is required when submitting the form online.
