The Core Program encompasses all features of community paramedicine and runs Monday to Saturday 7:00 am – 7:00 pm. Home visits are arranged to support patients with congestive heart failure, chronic inflammatory lung disease and diabetes. Community Paramedic Programs
Community paramedicine is a relatively new and evolving healthcare model. It allows paramedics to operate in expanded roles by assisting with public health, primary healthcare and preventive services to underserved, at-risk, and/or identified populations in the community.
Programs
Community Paramedic Long Term Care
This program is offered 24/7 and focuses on providing care to residents who are:
- On a long term care waitlist
- Soon to be on the long term care waitlist
- Opted to remain in their own homes
- Patients requiring supplemental Community Paramedicine services, beyond home and community care
- Citizens who may otherwise benefit from community paramedic involvement
The goal of this program is to:
- Provide supports to provide safe and prompt care for matters not requiring treatment at a hospital
- To ease pressure on long term care waitlists while allowing people to remain in their own homes
- To assist with caregiver burnout
- To decrease the hospital's emergency department admission of long term care bound patients
- To support hospital discharge for long term care bound patients
This program serves to connect with individuals facing daily challenges resulting from multiple situations. Our team works with other community services to provide follow-up and preventative care.

- Mental Health & Addictions
- Youth support
- Under housed
- Education
- Isolation through technology such as virtual reality
- Drug Strategy Task Force
- Palliative Care
- Devices to support those with health and well-being needs
- Support for clients who have improved and transitioned to periodic care

The Brant-Brantford Community Paramedic Program is offering a wish granting program that provides unique and personalized experiences to palliative and end of life clients within the County of Brant and the City of Brantford. The Wish Program grants palliative and end of life clients who are immobile or have care limitations, the opportunity to go out and do something they wouldn’t otherwise be able to do.
The Wish Program works with community partners, the clients themselves, and family members to make a client wish a reality. Wishes could be trips to the beach to watch the sunset, trips back to a family estate, or even to watch a live hockey game. Whatever the wish is, the Paramedic team will try our hardest to make it a reality. If the Program is unable to warrant a wish, the program is able to offer a parallel experience through Virtual Reality.
In order for a client to be eligible for the Wish Program, we ask they meet the following criteria:
- Must be palliative or receiving end of life care.
- Must be mentally competent or minimum GCS 13.
- Medically stable.
- Immobile or limited mobility.
- Client must be able to be moved, along with any supplied equipment.
- Valid DNR.
- Approval by Family Physician or NP, and BBPS Wish team.
Please note, that all submissions are reviewed on a case by case basis and some restrictions may apply.

Once a referral has been received, a Paramedic will be in contact and conduct an assessment to ensure the client meets the criteria.
